Abstract

Primary mucinous carcinoma of the skin is an uncommon malignancy of sweat gland origin. It is usually misdiagnosed clinically because of its benign appearance. On histological examination, it should be differentiated from metastatic mucinous carcinomas arising elsewhere in the body. We present a case of a 77-year-old male presenting with a swelling on the face, which was diagnosed clinically as a benign lesion, but on histological examination, it was diagnosed as mucinous carcinoma, and after thorough investigations, it was labelled as primary mucinous carcinoma of the skin.


Research Objective

To present a case of primary mucinous carcinoma of the skin and discuss its diagnostic challenges, particularly differentiating it from metastatic mucinous carcinomas.


Methodology

Case report detailing the presentation, clinical diagnosis, gross and histopathological examination, and immunohistochemical analysis of a 77-year-old male patient with a swelling on his face.

Discussion

Primary cutaneous mucinous carcinoma is a rare malignancy of sweat gland origin, often misdiagnosed clinically due to its benign appearance. It is locally aggressive with a tendency to recur but rarely metastasizes. Differentiating it from metastatic mucinous carcinomas from other sites (e.g., breast, salivary glands) is crucial and can be aided by microscopic and immunohistochemical parameters. Wide surgical excision is the treatment of choice, with chemotherapy and radiotherapy being less effective.


Key Findings

Histological examination revealed mucinous carcinoma with tumor cells in small clusters immersed in mucin lakes. Immunohistochemical analysis showed positive results for AE/AE3, CK7, Estrogen, and Progesterone receptors, and negative results for CK20 and p63. The lesion involved the deep margin.


Conclusion

Primary cutaneous mucinous carcinoma requires careful attention due to its rarity and the diagnostic challenges it presents, especially in distinguishing it from secondary mucinous carcinoma.
