Abstract

Objective: To assess the diagnostic accuracy of mammography and sonomammography in detecting breast cancer in women with palpable breast lumps, using histopathology as the gold standard.
Material and Methods: A total of 146 female patients above 35 years of age were included in the study. Mammography and ultrasonography were performed to characterize the lesions, and BI-RADS scores were assigned. Histopathological Biopsies were conducted based on the BI- RADS classification. The sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of both modalities were calculated.
Results: The study found that the mean age of the participants was 53.2 years, with a mean duration of breast lump of 8.1 months and a mean lesion size of 27.7 mm. The sensitivity and specificity of BI-RADS III cases on mammography were 6.09% and 47.9%, respectively, while for BI-RADS IV cases, they were 46.1% and 74.1%. BI-RADS V cases showed 100% sensitivity and specificity. The overall sensitivity, specificity, PPV, NPV, and accuracy of BI- RADS on mammography were 98.7%, 77.0%, 84.2%, 98.0%, and 89.0%, respectively.
Ultrasonography had similar results, with overall values of 98.8%, 83.8%, 89.2%, 98.1%, and 92.4% for sensitivity, specificity, PPV, NPV, and accuracy, respectively.
Conclusion: Both mammography and sonomammography were effective in detecting breast cancer in women with palpable breast masses. Ultrasound had better specificity and positive predictive value, making it a valuable tool for further characterization when mammography indicates malignancy, especially in resource- constrained settings.
