This case report is about a unique case of a 40-year-old male donor who were physically fit with no symptoms of any diseases appear to donate blood for his relative admitted in intensive care unit while pre-donation screening for transfusion transmitted disease on Roche c 404 immunoassay he was reactive for HIV, HBS and also positive for Plasmodium falciparum. This case was very concerning because donor was co-infected with three transfusion transmitted infections. This all diseases are very dangerous and lead to serious condition and only can be prevented to transfer by standard screening for TTI’S. The case indicted for future research into the prevalence’s of such Co-infection and their impact on overall health system in the region which is already facing issue like endemic of viral diseases.Keywords: HIV, Hepatitis B, Malaria, Co-infection, Blood Donation, Public Health
