Deeper understanding of root canal anatomy is fundamental for successful endodontic treatment. Two palatal roots in maxillary second molars, although uncommon, can be encountered in clinical practice. Advanced diagnostic tools like cone beam computed tomography (CBCT) plays a pivotal role in identification and management of this type of complex anatomy.
