Abstract

Objective: To evaluate the impact of in-house versus outsourced laboratory testing on turnaround time (TAT), cost, and quality in a small private laboratory setting.
Material and Methods: This retrospective observational study was conducted over four months (Jan–Apr 2024) at Reza Clinical Laboratory in collaboration with the Pathology Department, Quaid-e-Azam Medical College, Bahawalpur. A mixed-methods approach was used, analyzing quantitative data (TAT, error rates, costs) and qualitative data from interviews with five lab staff. Statistical and thematic analyses were performed, along with a cost-benefit assessment of both models.
Results: A total of 790 tests were processed, with 565 (71.5%) conducted in-house and 225 (28.5%) outsourced. In-house tests exhibited significantly faster TAT (2.99 hours vs. 24.11 hours) and lower error rates (1.02% vs. 1.97%). Cost analysis revealed lower per-test costs for in-house testing, although outsourcing was essential for specialized diagnostics. Qualitative insights highlighted in-house testing's superior control and quality, whereas outsourcing presented challenges such as logistical delays and communication gaps. However, outsourcing was viewed as necessary for the cost-effective management of low-demand tests.
Conclusions: In-house testing demonstrated clear advantages in TAT, quality, and cost for routine diagnostics. Outsourcing remains indispensable for specialized tests. A hybrid approach combining in-house capabilities with strategic outsourcing offers a balanced solution for small laboratories. Future research should examine how these findings apply to larger laboratory networks and different healthcare settings.
