Cardiac implantable electronic devices (CIEDs) have transformed the landscape of rhythm management since the introduction of the first pacemaker in 1958. Continuous technological advancements—from traditional transvenous systems to leadless pacemakers and conduction system pacing strategies like His bundle and left bundle branch area pacing—have significantly enhanced device safety, physiological pacing, and patient outcomes. Leadless pacemakers minimize infection and lead-related complications, while conduction system pacing preserves natural cardiac activation, particularly benefiting patients with heart failure or conduction abnormalities. Despite certain technical challenges and the need for long-term data, these innovations represent a major leap in precision cardiology and reinforce the role of CIEDs as both life-saving and life-enhancing tools in modern cardiovascular practice.
