Introduction: Through literature, a very weak relationship has been observed between calcium levels and severity of the stroke. However, deranged levels of serum calcium can lead to several cardiovascular and cerebrovascular events. The purpose of our study was to determine the correlation between serum calcium levels and NIHSS score at the time of presentation in patients with ischemic stroke.\nMethod: It was an observational study conducted at department of Neurology, Services Hospital, Lahore from 28th February 2022 to 28th August 2022. A total of 183 patients fulfilling the selection criteria were enrolled in the study from the medical emergency department. Then a blood sample was taken for routine investigations. All samples were sent to the laboratory of the hospital for assessments of Calcium levels. Reports were assessed, and Calcium levels were recorded. Meanwhile, patients were assessed for an NIHSS score by the researcher, and the severity of the stroke was assessed.\nResults: The mean serum calcium level among patients was 6.38 mg/dl±1.65. The mean NIHSS score for patients was 28.94± 8.03. A negative, strong, significant correlation was seen between calcium level and NIHSS score, i.e., r = -0.619, p-value = 0.00.\nConclusion: Results of this study showed a strong negative and significant correlation between serum calcium level and NIHSS score in patients presenting with the ischemic stroke.
