Introduction: Birth preparedness and complication readiness encourages pregnant women to use professional health services and receive timely information in order to prevent complications caused by three delays that lead maternal mortality. The aim of the study was to determine role and practice of nurses in birth preparedness and complication readiness among pregnant women. 
Methodology: A cross-sectional analytical study was conducted in two tertiary care hospitals in Peshawar using a universal sampling technique, a sample size of 76 nurses working in the gynae department were chosen. Data was collected using an adopted and validated tool. The SPSS version 22.0 was used to analyze the data.
Result:  The study rejected null hypothesis that there is a significant association between nurse’s role and practices in birth preparedness and complication readiness among pregnant women because the p-value (0.049) was less than the level of significance (0.05).
Conclusion: According to the study's findings, nurses' roles in birth preparedness and complication readiness depend on educating mothers about pregnancy danger signs, delivery planning, preventing delays in seeking skilled healthcare provider services, collaborating with other medical professionals during birth-related emergencies, providing information on family planning services, and instructing mothers in postpartum baby care techniques.
