This case report supports successful orthodontic management in a 13-year-old female patient with a retrognathic convex facial profile and Class II Division II malocclusion. The chief complaint included dental crowding, managed by adding a combination of rapid palatal expansion (RPE) and lip bumper appliance. RPE separated the midpalatal suture, thus facilitating maxillary transverse expansion, while the LB allowed for mandibular expansion by redirecting the oral forces and enhancing alveolar remodeling. Combining them solved severe crowding issues, improved arch coordination, and resulted in a broader and more harmonious smile. Radiographs, cephalometric analysis, and imaging showed a Class II Division II skeletal pattern with large overbite and severe dental crowding accompanied by impacted canines. With good cross-expansion, crowding resolution, occlusal improvement and good patient compliance the results following the treatment were satisfactory. Case particularly shows in treating complex malocclusions, stabilizing the transverse dimensions, and enhancing long-term functional and aesthetic outcomes.
